Nursery/Children's/Youth/VBS Ministry Volunteers

Please upload below the following documents in order to serve in any of these ministries:

1. Child Abuse History
2. Criminal Background Check
3. PA Disclosure form OR if you have lived outside of Pennsylvania within the last 10 years, your completed fingerprinting FBI clearance.

3 documents are required on this form no matter your residency situation.
These documents are good for 5 years from their issue date.
